In Northeast India, a priest and his daughter travel by boat to a temple during a festival. When the daughter is injured and her blood flows into the river, she releases Bak, a shape-shifting demon. Buck kills the daughter and impersonates her. The priest then traps Buck's soul in a vessel, forcing him to permanently take his daughter's place for his wife's sake. The scene shifts to a village where Selvi and her husband Santosh live with their children, Shakti and Saravanan Jr. Selvi's husband dies of a heart attack in the forest, and Selvi apparently hangs herself in their well. Seeing her dead mother, Shakti falls into a coma. Upon learning of the death, Selvi's brother Saravanan, a lawyer, visits the bereaved children with his aunt. In the village, the zamindar's granddaughter Maya, a doctor, is assigned to take care of Shakti. Soon, they start experiencing paranormal activities in the house.
Saravanan suspects that his sister Selvi's death is not a suicide and goes to the police station with Ravi, son of the village head. He is rudely dismissed by the inspector. Ravi then shows Saravanan a video of his late brother-in-law being chased by Swami before his death. Later, while they are in their jeep, a spirit attacks and Saravanan gets out to investigate. Meanwhile, a spirit that looks like Selvi attacks Ravi. Hearing Ravi's screams, Saravanan returns to find the jeep and Ravi drives away, leaving only the video camera behind, which once again shows footage of Swami. The next morning, Saravan finds the jeep crashed into a tree, with Ravi's body on top. Initially suspicious of the police, Saravanan clears his name using video camera evidence. The police, now convinced, help Saravan search Swami's lap, where they find photos of all the recent victims, Selvi, Santosh, Ravi and Shakti. Convinced of Swami's guilt, the inspector orders his arrest.
Saravanan finds out that Shakti, the inspector and late Ravi share the same birth date. He confirms with Maya that all three have the same red mole and asks her to protect the children. Saravanan confronts Swami to save the inspector. Meanwhile, a Northeast Indian priest arrives at Saravanan's house, claiming to be looking for his missing daughter. He tries to contact Shakti but is repelled by Selvi's spirit. Meanwhile, the inspector dies after being impaled by Vel, and Shakti wakes up from her coma with Selvi's spirit. During the interrogation, Swami reveals that Bak, an evil spirit capable of living in water and on land, had his spirit trapped by the priest and his powers diminished. Bak followed the villagers as they moved into the village, eventually retrieving his hidden soul. To gain absolute power, Baak aims to sacrifice Shakti, Inspector and Ravi. Buck kills Selvi's husband and takes his form to approach Shakti. Selvi, however, sacrificed herself to protect her children and returned as a spirit to protect against Bak. Swami warns Sarvanan to protect Shakti at all costs to prevent Buck from becoming immortal.
Saravanan tells about his regret for not supporting his aunt. Meanwhile, Baak attacks the police station, causing injuries, but Swami manages to drive him away. During this chaos, Maya is stopped by Selvi's spirit at the gate of the compound while trying to attend Shakti's birthday party. Saravanan sees blood stains on Maya's lab coat to which Selvi's spirit points. Upon testing the blood in the lab, they discover that Bak is impersonating Swami. After confronting the President, they find Swami's body in the trunk of the President's car. The president reveals that Swami went to the graveyard to destroy Selvi's spirit and her remains. Saravanan arrives too late as his sister's spirit disappears while pleading for Shakti's safety. Bak then reappears to claim power, but Maya escapes with him at the temple festival. There, Saravanan battles Bak inside a giant Asura sculpture. At the climax of the festival, a divine spirit destroys both the sculpture and the bak.
With the mess resolved, Saravanan assures Maya that the palace will be converted into a hospital to continue serving the village. Outside, the children play throwball with their mother's spirit, indicating that she watches over them constantly.
